What name do you put on a married women's place card, Mrs. John Doe or Mary Ann Doe?

For the escort card (the card that tells each person or couple at which table they will be sitting), you should put Mr. and Mrs. John Doe.

For the place card (the card put at each place setting letting people know in which seat they will be sitting), you should put Mr. John Doe for the husband and Mrs. Mary Ann Doe for the wife.
